This is a Lensbaby Composer Kit for Canon EF.

This kit is in excellent condition and shows only small signs of use. Comes with a Lensbaby Composer, a Double Glass Optic with container, Aperature Disks (2.8-22), a 1.6x Telephoto Lens 37mm, and a 0.6x Wide-Anlge Lens 37mm. The telephoto has one spec of debris/dust but other optics are clean of any scratches, fungus, haze, or separation.
